Dependent Day Care Assistance Plan (DCAP) Open Enrollment
The enrollment period starts November 20, 2008 and ends December 12, 2008. The
program is a federally administered program, so it is imperative that all interested
employees enroll prior to December 12, 2008.
TO ENROLL OR TO CONTINUE YOUR CURRENT PARTICIPATION:
o If you want to participate in DCAP, you must enroll by December 12, 2008.
o TO ENROLL: Please complete the enclosed enrollment form and return it in the
enclosed envelope or mail it to WageWorks at 2250 Butterfield Drive, Suite 100,
Troy, MI 48084. If you need a form contact your UAW Ford ESSP Representative.
o If you have any questions, please feel free to contact the WageWorks Service
Center at 1-877-924-3967, 8am-8pm Eastern Standard Time, Monday through Friday.
